Community Health Survey

Your input will be used to shape community health improvement planning and future funding invested in community health and well-being.

 

 

Who wants to know about the community and health needs of Central Oregon?

The Central Oregon Community Health Survey is led by the Central Oregon Health Council in partnership with St. Charles Health System, Crook County Health Department, Deschutes County Health Services and Jefferson County Public Health. It happens every five years as part of a Regional Health Assessment, a report that identifies the key health needs, weaknesses and strengths of a community.

Who should take the survey?

Everyone who lives in Central Oregon should take the survey. We would like to hear from all Deschutes, Crook, Jefferson, northern Klamath, and Warm Springs community members.
